SUMMARY: The Social Worker is responsible for the counseling of assigned clients as well as their family members. The Social Worker acts as a resource person to other program staff and to the facility in the area of counseling, family dynamics/interactions. The Staff Social Worker is under the direct supervision of the Administrator. DUTIES PERFORMED: * + Conducts psychosocial history to determine pre\-injury issues which need to be addressed. + Establishes and carries out individualized treatment plan upon completion of psychosocial history if applicable. + Provides counseling to clients, families and/or significant others. + Develops and implements family and children support groups as needed. + Schedules, arranges and moderates the program for family education group. + Participates in team rounds and family conferences as scheduled through case manger. + Maintains records and prepare reports according to facility procedures. + Participates in and documents internal and external professional educational activities to enhance personal and professional growth. + Performs public relations task as directed by supervisor. + Coordinates client's schedule with physical therapy, occupati...
