**Responsibilities May Include:** * Participate as needed with in office or telephonic case conferences with the Interdisciplinary team. * Coordinate Assessment findings and DC planning with all members of the HH treatment team. * Attend required training on HH clinical programs * Attend all in\-services and training related to HH regulatory changes and PT practice related to our current HH polices and procedures. * To instruct other health care team personnel in certain phases of physical therapy with which they may work with the patient. * Counsel patients to help them understand their disorder. * Instruct the patient and family in care and use of physical therapy devices and all aspects included in the patient’s plan of care. * Work with patients of all ages. * Provide physical therapy services as prescribed by a physician which can be safely provided in the home. * Evaluate patients by applying diagnostic muscle, nerve, joint and functional abilities tests (PT only). * PTA will progress a patient using appropriate treatment plan outlined and established by a PT. * Work with teachers, physicians, psychologists, social workers, and other helping professionals to address patient m...
