2CHealth Solutions is currently looking for a full\-time Chronic Care Case Manager to join our growing organization. We specialize in care for patients with chronic diseases. Your primary duty would be to work remotely as a Case Manager. The Case Manager, who works as a Care Coordinator, is a trained healthcare professional who helps to address patients' concerns and manage their chronic diseases through Chronic Care Management (CCM), Principal Care Management (PCM), and Remote Patient Monitoring (RPM) programs. Responsibilities, but not limited to: * Monitoring and coordinating treatment and care plans. * Providing care management and coordination services through **CCM, PCM, and RPM** programs. * Conducting patient outreach, follow\-ups, and health assessments as required. * Educating patients about their disease condition and helping them understand their care plans. * Monitoring RPM data and identifying changes or concerns that may require follow\-up or escalation. * Connecting patients with healthcare providers, and evaluating their progress. * Updating patient\-related health information, insurance and financial information. * Ensuring all immediate patient concerns and reque...
