Medical Assistant – Patient Monitoring (RPM) \& Chronic Care Management (CCM) Job Type: Full\-Time Position: Medical Assistant – RPM / CCM / Patient Care Work Setting: In\-Office Call Center About the Position We are seeking an experienced, compassionate, and highly professional Medical Assistant to join our growing Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) team. This is a patient\-facing position in which you will communicate with patients primarily by telephone, review remote monitoring activity, conduct structured health assessments, encourage patient compliance, document interactions, and escalate clinical concerns according to established protocols. The ideal candidate is comfortable speaking with patients throughout the day and knows how to combine professionalism with empathy. You should be organized, dependable, detail\-oriented, and capable of building rapport with older adults and patients managing chronic health conditions.
