Responsibilities/Job Description: Fairview has an immediate opening for an In Clinic Visit Facilitator to join our Organ Transplant Care team in Minneapolis, MN. This is a 0\.8FTE (**64 hours** per two weeks pay period), benefits\-eligible opening. **Clinic Hours of Operation** are Monday–Thursday: 7:00AM–6:00PM and Friday: 7:00AM–5:00PM. Visit Facilitators are a valuable member of our care team and is responsible for supporting in person clinic visits, including communicating with patients prior to their visit and helping guide patients through the care experience. The visit facilitator gathers pertinent medical information from the patient and documents the information into the patient's medical record. This role also helps to ensure the exam rooms are stocked, equipment and supplies are available and assists with tasks that support the effectiveness and efficiency of the team.
