This position is for a skilled nursing facility under new ownership in Stamford. Stamford Nursing and Rehabilitation has both short\-term and long\-term patients. This facility has **an in\-house therapy team with smaller daily case loads** withan emphasis on optimizing time with patients. **We have flexible scheduling!** We are dedicated to creating a family\-like atmosphere for our residents, and our employees look forward to coming to work and delivering the highest quality of healthcare services possible. **Speech Therapist Job Responsibilities:** * Develop and implement a Speech Therapy plan of care for facility residents and short\-term patients. * Assist as needed with patient screens for Speech Therapy. * Communicate progress and any barriers toward SLP goals with interdisciplinary team members. * Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements. * Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods. * Provide and document patient, family, and caregiver training regarding each speech therapy patient’s specific plan of care and ...
