We are so glad you are interested in joining Sutter Health! **Organization:** SHSO\-Sutter Health System Office\-Bay**Position Overview:** This role will support the Multidisciplinary Inbasket Support Team (MIST) Works independently and in collaboration with physicians and other health care providers across the health care continuum to ensure appropriate medication therapy for individual patients within the Sutter system. Performs medical renewal assessments remotely for physician and care team in\-baskets. Prescribes medications for renewals as well as adjust medications as needed for chronic disease states under a collaborative practice agreement. Is responsible for providing clinical assessment regarding patient home readings and support providers with medication questions.**Job Description**: **EDUCATION** * PHARMD\-Graduate of an accredited pharmacy school **CERTIFICATION \& LICENSURE** * PHARMR\-Current registration or Registered Pharmacist within 120 days * ASHP\-Post Graduate Year 1 ASHP General Pharmacy Residency **TYPICAL EXPERIENCE:** * 3 years recent relevant experience **PREFERRED EXPERIENCE:** * Practicing autonomously under a collaborative practice agreement. * Comfo...
