Overview: **JOB SUMMARY:** The Consent Coordinator supports HealthDrive’s Care Management Program by traveling to skilled nursing and post\-acute care facilities across company markets to educate residents and obtain informed program consent. This highly mobile, field\-based position requires frequent air travel, overnight stays, and independent scheduling. The coordinator explains program services clearly and compassionately, ensuring each resident has the opportunity to make a voluntary, informed decision. Accurate documentation, daily activity reporting, and productive collaboration with facility and HealthDrive teams are essential to successful program enrollment. **ACCOUNTABILITY:** Responsible for independently executing assigned facility consent visits, accurately recording resident interactions and consent outcomes, and submitting complete daily activity reports. Owns assigned enrollment activity and timely escalation of barriers while respecting resident choice and all applicable consent and privacy requirements.
