**Program:** Coordinated Family Support (CFS) **Position Type:** Full\-Time / Part\-Time **Pay:** Starting at $25\.00 per hour **Reports To:** CFS Program Manager / Administrator POSITION SUMMARY Creative Concepts Living Services (CCLS) is seeking a compassionate, organized, and resourceful CFS Case Worker / Family Resource Specialist to provide Coordinated Family Support (CFS) services to adults with developmental disabilities living in their family home. The CFS Case Worker works directly with individuals and families to implement assigned service plans, navigate community resources and public benefits, address barriers, coordinate referrals, advocate for needed supports, and complete appropriate follow\-up and documentation.
