Responsibilities: The clinical pharmacist is responsible for leading the development and implementation of clinical pharmacy services that enhance the participant experience and support overall organizational performance. This role establishes and maintains a participant\-centered pharmacy practice within primary care, focusing on medication management, therapy optimization, and alignment with individual goals of care. The pharmacist works collaboratively with primary care providers, advanced practice providers, center staff, and the pharmacy team to ensure safe, effective, and cost\-effective medication use.
