Perpetual Care Home Health is currently seeking a compassionate, skilled and dedicated Home Health **Physical Therapist (per diem)** to join our team and help improve patients' mobility, function, and overall quality of life in the comfort of their homes. * Service Area: San Mateo County * Position: Physical Therapist \- Per Diem * Compensation: $100\-$170 per visit, depending on the type of visit Job Description Summary: The Physical Therapist employed through the organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement. Essential Job Functions/Responsibilities 1\. Provides physical therapy services to patients according to a written physician’s plan of care. This may include, but not limited to: * Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
