**Reports To:** *Clinical Manager* Our home health care agency is expanding and searching for dedicated Home Health RNs for FT and PRN positions. Come join an amazing agency that values their employees. **Responsibilities** * Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team. * Complete evaluation tasks, including reviewing medication and vital signs. * Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated. * Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates. * Educate patients and caregivers to promote independence and improve health outcomes, including training in self\-care techniques. * Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan. * Supervise and support LPNs to ensure high standards of care. * Perform additional duties as needed to support patients and team operations. **Qualifications** * Current Registered Nurse (RN) license in the state of...
