**Overview**The Dartmouth\-Hitchcock Pharmacy seeks a clinical pharmacist to join our Hematology Oncology team in Manchester, NH. In this role you will engage patients and healthcare professionals to efficiently and effectively maximize therapeutic outcomes and will be responsible for both operational and clinical mangement of patient care. The selected candidate will have Oncology and/or sterility compounding experience. This is a full\-time, benefitted position working Monday\-Friday, 8:30 AM\-5:00 PM. *Keywords: Dartmouth\-Hitchcock Pharmacy; Pharmacist;* **Responsibilities*** Review patient drug therapy and care plans with medical staff and other healthcare professionals. * Expand existing clinical programs including chemothery teaches. * Clinical liason for medical staff including provider and nursing colleagues. * Proficient in USP797\-800, dispense prep, and camera integration in the clean room. * Completes a comprehensive medication review to make evidence\-based recommendations to ensure medications are necessary, safe and cost effective. * Identifies and records suspected adverse drug reactions, adherence gaps to medications and barriers to patient adherence. * If not pra...
