**Bargaining Unit:** Outpatient Clinics \- EAP **Rate of Pay:** $72\.79/hour \+ DOE (Base Pay of $64\.71/hour includes 12\.5% per diem premium) **Summary** Performs the nursing process through assessment, planning, implementation and evaluation of safe therapeutic care to patients in the Multispecialty Clinics by performing the following duties. **Essential Duties and Responsibilities** * Assesses the physical and psychosocial needs of patients. * Implements and evaluates individual plan of care. * Evaluates the effectiveness of interventions. * Identifies and provides education to patient and/or significant other. * Coordinates resources for complete and effective patient care, primarily focused on electronic health record in basket management. * Communicates effectively using Electronic Health Record (EHR) messaging platforms. * Participates in departmental growth and development. * Rotates and covers in\-basket management between departments as assigned/required. * Pursues professional growth. * Follows patient care standards. * Explains medications, in\-basket concerns and solutions to patient to gain cooperation, understanding, and allay apprehension. * Maintains awareness of ...
