Responsibilities/Job Description: Fairview has an opening for a Team Coordinator to join our Primary Care Clinic in Minneapolis, MN. This is a 1\.0FTE (**80 hours** per two week pay period), benefits\-eligible opening. **Clinic Hours of Operation** are Monday–Thursday: 6:30AM\-6:30PM and Friday 6:30AM\-5:30PM. NO WEEKENDS! As a member of the care team the team coordinator performs quality patient\- and family\-centered care principles through performing a variety of high\-level functions within a care team to support the needs of the healthcare team and patients/families. The team coordinator manages workflow by coordinating and maintaining provider schedules, uses a variety of communication tools and systems to handle medical information requests and updates, communicates normal test results either through the direction of the RN or provider or by following established protocols, and works collaboratively with the assigned unit while handling multiple tasks, interruptions, and changes.
