The Patient Care Assistant is an entry\-level position responsible for providing administrative and clinical support within the Memory Clinic. The primary focus of this role is the administration of standardized cognitive assessments and the documentation of Chronic Care Management (CCM) activities within the eClinicalWorks (eCW) platform.
