**Whittier Rehabilitation Hospital \- Transitional Care Unit/SNF \- Haverhill MA** RN Charge Nurse $42\-$47/hour 20 Bed Transitional housed within our Rehabilitation Hospital Regular Schedule will be M\-F days (40 hours) with availability to pick up extra shifts We are seeking a Charge Nurse to oversee and lead our clinical team on our Transitional Care Unit housed within our beautiful Rehabilitation Hospital on Ward Hill in Haverhill Ma. **Essential Duties**: To provide exceptional care to all residents including but not limited to:* Under the direction of the Unit Manager, manages the nurse desk and clinical staff on the unit * Obtain report from nurse being relieved and records sufficient information to implement appropriate follow up action as necessary * Provides report to nurse coming on duty, including sufficient information for follow\-up action if necessary * Conducts patient rounds, reporting problems to nursing supervisor and initiating corrective action * Administers and documents medication, enteral nutrition and treatment per physician order and accurately records all care provided and observations daily * Performs comprehensive assessments of each resident on unit * ...
