**Position Summary** The Clinical Pharmacist provides medication management services, supports providers in clinical decision making, and helps improve patient outcomes through safe and effective medication use. This role collaborates with physicians, advanced practice providers, nurses, and other healthcare professionals to optimize medication therapy. ### **Essential Duties and Responsibilities** * Review medication regimens for safety, effectiveness, interactions, and appropriate dosing. * Provide medication therapy management (MTM) and patient counseling. * Identify and help resolve medication\-related problems. * Monitor patients' responses to medication therapy and recommend adjustments to prescribing providers. * Assist with chronic disease management, including diabetes, hypertension, and other ongoing conditions. * Support medication reconciliation during care transitions. * Educate patients about medication adherence, side effects, and proper medication use. * Collaborate with physicians, nurse practitioners, and other clinical staff. * Support formulary management, medication safety initiatives, and evidence\-based prescribing practices. * Document clinical interventions...
