**Job Description Summary** We are seeking a dedicated Multi\-State Registered Nurse to join our dynamic Nurse Triage team, responsible for delivering immediate, high\-quality advice on incoming calls and actively participating in the after\-hours on\-call rotation. In this versatile role, you will champion patient care by managing post\-acute and hospitalist outreach, facilitating prescription refills, monitoring patients in specialized tracking programs, and supporting RN Care Management teams with assessments and follow\-up calls.
