Under the direction of the Clinical Manager and the Pharmacy Director; optimizes patient medications therapy by providing comprehensive clinical pharmacy services. Works collaboratively with the health care team to accomplish goals. Must be proficient in all nine patient care skills: knowledge of disease; knowledge of drug therapy; knowledge of non\-drug therapy; knowledge of laboratory and diagnostic testing; communication skills; patient monitoring skills; physical assessment skills; drug information skills; and therapeutic planning skills. JOB REQUIREMENTS Minimum Education B.S. Pharmacy or Pharm D. or Master of Pharmacy Minimum Work Experience Three years as a licensed pharmacist with enhanced clinical responsibilities Required Licenses/Certifications Active registration as a licensed pharmacist in the state of Texas. Sterile Compounding Certification. ACLS or PALS Board Certified Pharmacotherapy Specialist (BCPS) or specialty certification in area related to assigned duties. Registration as a preceptor with TSBP.
