Job Types: Full\-Time Pay Type: $52 to $55 Hourly (not per visit/per diem) Location: Driving to patients' homes Service Coverage Area: Orange County #### **Essential Job Functions/Responsibilities:** * Visits patients to assess and evaluate patient’s status. * Initiates the plan of care and makes revisions as needed. * Administers medications and treatments as prescribed by the physician. * Provides health care instructions to the patient \& family. * Completes accurate clinical notes in electronic EHR. * Works in cooperation with the family/caregiver. #### **Position Qualifications:** * CA RN License * CPR/BLS certification * CA Driver License * At least 1 year of Nursing Home Health Experience #### **About Acacia HH\&P of Southern California LLC** The Acacia Promise!
