Summary: **Hours: 40 hrs/wk; Rotating Shifts, Weekdays, Weekends and Holidays** The Department of Pharmacy is seeking a highly motivated, energetic, and dynamic individual to serve patients across the transitions of care continuum. The Pharmacy Transition Specialist working with the Central Outpatient Pharmacy team will be responsible for logistically organizing, filling, collaborating and/or delivering required discharge medications to patients and/or families or caregivers. The Specialist will ensure the successful hand\-off to the appropriate medications to enable the improvement of the patient’s health management and experience. This position qualifies for our Pharmacy Technician Incentive Program, which includes $5000 in commitment incentives spread over a two\-year period.
