**Physical Therapist, PT \- PRN, Per Diem** **Position Summary** -------------------- The Physical Therapist (PT) is responsible for evaluating, planning, and implementing individualized physical therapy treatment programs for residents in a Skilled Nursing Facility (SNF). The PT works collaboratively with an interdisciplinary team to improve residents' mobility, functional independence, safety, pain management, and overall quality of life while complying with all federal, state, and facility regulations.
