**SUMMARY** As a Physical Therapist – Rehab, your ability to design, plan, and implement personalized physical therapy intervention greatly enhances patient function and overall quality of life. **STATUS:** Per Diem **LOCATION:** Unity Hospital \- 1555 Long Pond Rd, Rochester, NY 14626 **DEPARTMENT:** Inpatient Acute Care **SCHEDULE:** Day shift, 8\-hr. shifts; 8:00 am \- 4:30 pm; weekend rotation requires at least one (1\) weekend per month **RESPONSIBILITIES** * **Treatment \& Planning.** Meet patient goals and needs by providing quality, evidence\-based care; assess, develop, and implement an individualized treatment plan; help the patient accomplish the treatment plan and accept therapeutic devices by administering manual exercises; instruct, encourage, and assist patients in performing physical activities, such as non\-manual exercises, ambulatory functional activities, and daily\-living activities * **Discharge Management.** Assure continuation of therapeutic plan following discharge by designing home exercise programs; instruct patients, families and caregivers in home exercise programs; recommend and provide assistive equipment; recommend outpatient or home health follow\-u...
