Description: Accommodate, Restore \& Collaborate, LLC is seeking a full\-time Physical Therapist at Meadowbrook Nursing \& Rehabilitation in LaGrange, IL. We are looking for a PT to prevent, assess, diagnose, and treat gait, mobility, and function in adult patients at this site. The job duties include, but are not limited to, the following: * Performing evaluations/plans of care and providing treatment accordingly. * Appropriately and effectively completing documentation. * Screening residents and individuals who may benefit from therapy. * Communicating patient status and needs to the patients, family, caregivers, or other members involved with patient care. * Directing and supervising the work of assistants as applicable. * Expanding and enhancing self, in both clinical and professional knowledge through associations and continuing education. * Providing in\-service training sessions or other educational presentations to members of the interdisciplinary team as needed. * Supporting and participating in department operations/meetings.
