The PT educates patients and their family members and ensures the safety of the patient. The PT is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. **KEY RESPONSIBILITIES** * Provides skilled physical therapy in a patient’s home as ordered by the attending physician * Observes, records, and reports patient reactions, changes, and progress to attending physician and any appropriate agency staff * Performs OASIS assessments to develop an individualized plan of care and makes adjustments as needs change * Develops plan of care ...
