**JOB TITLE:** Supports and Services Specialist**DEPARTMENT:** Business Operations Department**REPORTS TO:** Manager of Person\-Centered Services**FLSA CLASSIFICATION:** Non\-Exempt **JOB PURPOSE:** The Supports and Services Specialist is responsible for leading the organization’s admissions, referral development, and transition processes. This role serves as the ambassador of the organization, ensuring appropriate placement into programs, stakeholder engagement satisfaction, and sustainable growth. The Supports and Services Specialist also assists in the development and implementation of Person\-Centered Plans (PCP) by utilizing the Council on Quality and Leaderships (CQL’s) Personal Outcome Measures (POM) to identify meaningful outcomes, build individualized goals and PCP’s, and collect data to improve both service delivery and organizational outcomes. Through collaboration and commitment to person\-centered excellence, this position promotes quality person\-centered planning and helps to ensure services are responsive to each individuals unique goals, preferences, and needs.
