**Schedule:** Monday \- Friday **Pay Range:** $42\.85 \- $68\.45 Depending on Years of Experience **JOB PURPOSE:** The Senior Manager of Care Coordination plays a critical role in advancing quality, outcomes\-driven, and accessible patient care within a value\-based care framework. This position is responsible for planning, directing, and aligning the efforts of a diverse care team—including social workers, medical assistants, registered nurses, and program assistants—to ensure coordinated, efficient, and patient\-centered services. Working closely with the Senior Director of Care Coordination and Vice President of Population Health and Chief Quality Officer, the Senior Manager ensures that workforce productivity and care delivery models are strategically aligned to organizational goals.
