**POSITION SUMMARY** The Care Partner(AL)assists Assisted Living residents with activities of daily living (ADLs) and provides companionship to ensure an inclusive living environment. **RESPONSIBILITIES** Include but are not limited to the following: * Provides companionship to residents of the Assisted Living facility to promote emotional well\-being. * Assists residents with ADLs tailored to the individual needs of each resident. * Report all incidents/accidents/potential liabilities to Health Services Director and participate in required post incident shift huddles when requested. Monitors and reports any changes to residents’ health. * Engage residents in social activities and provide companionship; assist Life Enrichment Director as needed. * Assist with grooming before meals, serve meals while aiding with eating and drinking, and verify resident’s identification prior to serving. Monitor the dining room, as necessary. * Maintain resident documentation, charting required information, including any resident changes. Add appropriate residents to Alert Charting. * Assists with personal hygiene, mobility, toileting, meals, housekeeping, and medication as required per care plan. * ...
