**Quick Facts** =============== * **Pay:** Negotiable based on experience, knowledge, skills, and abilities * **Location:** Chiefland, Bronson, Trenton * **Schedule:** PRN, successful candidates will have 2\-3 days of availability weekly from 7:00a\-4:00p * **Employment type:** W\-2 * **Mileage:** Currently $0\.60/mile * **Reports to:** Branch Director **What You'll Do** ================== * Assess each new patient's condition, history, and home care needs * Build the care plan with the patient and family * Update the care plan as the patient's status changes * Administer medications and treatments as prescribed * Teach patients and families how to manage care at home * Coordinate with physicians and send updates at least every 60 days * Coordinate with community health partners involved in the patient's care * Plan and carry out discharge when the patient is ready * Arrange any equipment or services the patient needs * Check in on home health aide care every two weeks **Qualifications** ================== * Graduate of an accredited school of nursing.
