We are currently seeking a full time/ part time Physical Therapist (per diem) for our 2nd outpatient private practice located in Hackensack, NJ to be responsible for providing professional rehabilitation therapy to patients through evaluation, development, and implementation of individual goal\-oriented therapy programs. The incumbent will also be responsible and accountable for the following duties: ESSENTIAL JOB FUNCTIONS: * Develop an individualized treatment program based on the evaluation findings focusing on time\-oriented, functional and measurable short and long\-term goals. * Complete patient evaluation within the specified period; ensure it contains data that is reflective of a complete, holistic assessment of the client s condition. * Prepare appropriate and precise documentation, such as progress notes, reassessments and discharge summaries according ...
