**Registered Nurse (RN) – Assessment \& Care Plan Coordinator** **Daily Grace Adult Day Health Services, Inc. – Macon, GA** **PRN / Part\-Time** Daily Grace Adult Day Health Services, Inc. is seeking an experienced, dependable, and highly organized **Registered Nurse (RN)** to serve as our **RN Assessment \& Care Plan Coordinator**. This is primarily an **assessment, care\-planning, and clinical documentation position**, rather than a traditional bedside nursing role. The RN will be responsible for ensuring participant nursing assessments, supervisory visits, person\-centered care plan reviews, and related clinical documentation are completed accurately and within required timeframes. We are specifically seeking an RN who is **strong in clinical documentation, attention to detail, organization, and independent follow\-through**.
