**Job Summary** Under the supervision of the Program Manager, the Community Resource Coordinator is responsible for assisting clients in gaining access to workforce development resources and increasing access to hunger relief and healthcare services for underserved populations in San Bernardino and Riverside Counties. This position includes providing support for employment readiness, skills training, CalFresh enrollment, nutrition education, healthcare outreach and enrollment services such as Medi\-Cal, Covered California, and Enhanced Care Management (ECM). The Community Resource Coordinator works collaboratively with internal teams and external partners to empower clients through education, enrollment assistance, and direct services. **Essential Duties and Responsibilities** To successfully fulfill this role, the individual must perform the following duties to a satisfactory standard: **Case Management** * Conduct comprehensive client intake and needs assessments to develop individualized service plans, coordinating all necessary referrals and support resources. * Manage and maintain accurate client schedules, including booking appointments, arranging follow‑up visits, sending ti...
