**EDUCATION \& EXPERIENCE** **Minimum Qualifications:** * Bachelor of Science in Pharmacy or Doctor of Pharmacy degree. * Completion of an ASHP accredited post\-graduate year 1 residency program or 3 years of related experience. **LICENSES, REGISTRATIONS OR CERTIFICATIONS** **Required:** * Licensed to practice pharmacy in the state of Texas or eligible for licensure from the Texas State Board of Pharmacy. * Must be licensed with the Texas State Board of Pharmacy within 90 days of hire. **Preferred Qualifications:** * Doctor of Pharmacy degree * Completion of an ASHP accredited post\-graduate year 2 residency program in Transplant or Oncology * Experience as a Transplant or Hem\-Onc Pharmacist **JOB DESCRIPTION** The Clinical Practice Pharmacist provides advanced medication therapy management and direct or consultative patient care services across assigned practice areas.
