**Home Health Nurse** **RN** **he Center for Elders' Independence** is a PACE (Program of All\-Inclusive Care for the elderly) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly, and promoting autonomy, quality of life and the ability of individuals to live in their communities. **The Position:** Under the direction of Manager of Care Coordination and Home Health Nursing, the Home Health Nurse (HHN) provides high\-quality, compassionate, and patient\-centered nursing care to participants at the CEI clinic and at their homes. This role ensures the delivery of safe, effective, and individualized care, following the participant's care plan, physician's orders, and regulatory standards. The HHN communicates effectively with all members of the clinic team and other program staff to ensure that participants are receiving care that is appropriate and adheres to the participants' plan of care.
