**Job Description:** The Clinical Program Coordinator assesses, plans, implements, and coordinates treatment processes from diagnosis through follow up, specifically guides patients through the collaborative process with all medical care providers and ancillary support programs and provides educational support along the continuum. 2 part time positions, 20 hours a week, day shift (Monday \- Friday 0730\-17:00\) No nights/weekends/holidays. One day per month we work 7:30PM\-9:30PM to prep for conference with the provider on Teams. **Essential Functions** * Provides patients with information regarding available resources and options to facilitate shared decision making with their care providers * Provides patient and community education * Ensures optimal patient care using evidence\-based, clinical best practice models, appropriate staffing and technologies and continuous process improvement measures * Involved in patient engagement efforts and monitors patient perception of the quality of care delivered * Coordinates care across the continuum, including planning and implementing clinical protocols and practice management guidelines, monitoring care, and serving as an expert resource...
