**POSITION SUMMARY** The Physical Therapist is responsible for evaluating, planning, and implementing individualized physical therapy programs for residents in a skilled nursing facility. The goal is to help residents achieve their highest level of physical function and improve their quality of life. The Physical Therapist will work collaboratively with other healthcare professionals to ensure comprehensive care.
