**Job Summary and Responsibilities** As our RN Care Coordinator, you will oversee the progression of care and discharge planning for identified patients, promoting quality and cost\-effective outcomes while adhering to all hospital policies and regulations. Every day you will coordinate, communicate, and collaborate with utilization management, nursing, physicians, ancillary departments, insurers, and post\-acute providers to progress care toward optimal outcomes at the appropriate level. You will also advocate for patients and families, identifying and addressing barriers to care transitions, and striving to enhance the patient experience.
