**EDUCATION \& EXPERIENCE** **Minimum Qualifications:** * Bachelor of Science in Pharmacy or Doctor of Pharmacy degree. * Completion of an ASHP accredited post\-graduate year 1 residency program or 3 years of related experience. **Preferred Qualifications:** * Doctor of Pharmacy degree * Completion of an ASHP accredited post\-graduate year 2 residency program for specialty areas or job. * Specialty Pharmacy expereince preferred **LICENSES, REGISTRATIONS OR CERTIFICATIONS** **Required:** * Licensed to practice pharmacy in the state of Texas or eligible for licensure from the Texas State Board of Pharmacy. Must be licensed with the Texas State Board of Pharmacy within 90 days of hire **Preferred:** * Board certification through the Board of Pharmacy Specialties. **JOB SUMMARY** The Clinical Practice Pharmacist provides advanced medication therapy management and direct or consultative patient care services across assigned practice areas.
