About the Role We are seeking a compassionate, organized, and detail\-oriented **Adult Foster Care (AFC) Case Manager** to join our team serving the South Shore region of Massachusetts. In this field\-based role, you will work closely with participants, family caregivers, and multidisciplinary healthcare teams to ensure high\-quality, person\-centered care in a home setting. Key Responsibilities * Conduct regularly scheduled in\-home visits to evaluate participant health, safety, and overall well\-being. * Collaborate with Registered Nurses (RNs) and primary care physicians to build, implement, and review multi\-disciplinary care plans. * Support caregivers by providing resources, training, advocacy, and guidance. * Maintain strict compliance with MassHealth Adult Foster Care regulations, completing timely and thorough clinical documentation and progress notes. * Connect participants to local community resources, medical supply programs, and specialized transportation services. Service Area This position requires travel throughout Norfolk and Plymouth Counties.
