**Job Description Summary** ‎ We are seeking Physical Therapists (PT) to join our Home Health Team in Charlotte, Collier, and Sarasota Counties. In this role, there is a real impact in the comfort of patients’ homes. As part of our compassionate, high‑performing care team, you will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to improve mobility, strength, balance, and functional independence. You will collaborate with an interdisciplinary care team, educate patients and families on rehabilitation and injury prevention strategies, maintain accurate clinical documentation, and ensure compliance with all regulatory standards to deliver high\-quality patient care and achieve optimal outcomes. This is a Per Diem (PRN) position with variable time and hours can vary from week to week based on staffing needs, patient volume, call\-offs, vacations, or organizational requirements.
