**Position Overview** ===================== United Health Services (UHS) is seeking an Ambulatory Clinical Pharmacist to provide comprehensive medication therapy management and disease state management services within the ambulatory care setting. As an integral member of a multidisciplinary healthcare team, the pharmacist collaborates closely with physicians, advanced practice providers, nurses, and other healthcare professionals to optimize medication use and improve patient outcomes. Working under collaborative practice agreements, the Ambulatory Clinical Pharmacist delivers direct patient care through in\-person and telehealth encounters, assessing medication regimens for appropriateness, effectiveness, safety, tolerability, and adherence. The pharmacist is responsible for identifying, resolving, and preventing medication\-related issues; establishing individualized treatment goals; and managing, monitoring, and adjusting medication therapies to achieve optimal clinical outcomes.
