**Registered Nurse (RN) – Hospice** =================================== **Position Summary** -------------------- The Registered Nurse (RN) provides nursing care to hospice patients in their place of residence and coordinates care with the Interdisciplinary Team (IDT), physicians, patients, families, and referral sources. The RN is responsible for assessing patient needs, implementing the plan of care, and ensuring quality end\-of\-life care. **Responsibilities** -------------------- * Perform initial and ongoing patient assessments * Develop, implement, and update individualized plans of care * Coordinate care with the IDT, physicians, patients, and families * Provide patient and caregiver education regarding disease process, medications, end\-of\-life care, and safety * Determine visit frequency based on patient needs and acuity * Supervise Hospice Aides and LVN/LPN staff * Document clinical findings, interventions, and patient progress accurately * Report changes in patient condition to physicians and team members * Practice infection control and standard precautions * Participate in QAPI and hospice in\-service education programs * Provide on\-call services as assigned * Promot...
