**Summary** As a Physical Therapist with Access Health Care Physicians, you will perform and document an initial exam on every incoming patient in order to identify problems and determine plan of care and implement different interventions. You will also identify and document goals as well as anticipated progress and plans for reevaluation. **Essential Duties and Responsibilities** include the following. Other duties may be assigned. * Testing and measuring patients’ strength, function, sensory perception, functional capacity and respiratory and circulatory efficiency * Planning, preparing and carrying out individually designed programs of physical treatment to maintain, improve or restore highest function performance * Instructing patients and families in home exercise program and safety precautions to be continued at home * Attending staff meetings and facility meetings * Participating in continuing education classes **Qualifications** To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
