The Physical Therapist is a qualified professional who directs, supervises, evaluates and provides direct care to patients and provides physical therapy services to patients in the home according State and Federal regulation under the direction of the attending physician. EEO/AA/Disability/Veteran Responsibilities: * 1\. Comprehensive evaluation of the patient's level of function and development of the therapy plan of care. * 2\. Participation in case management conferences and documentation thereof. * 3\. Instruction of patient, caregivers, PTAs, and other agency clinical staff in the patient's treatment plan including follow up supervision. * 4\. Selects and administers diagnostic and therapeutic techniques and materials. * 5\. Supervision of Physical Therapy Assistants.
