The Hospital Liaison/Marketer serves as the primary point of contact between our nursing homes and families, hospitals, referral sources, and the community. This role is responsible for facilitating and coordinating patient referrals and admissions across our nursing homes through strong relationship\-building with healthcare professionals—including Social Workers, Case Managers, Discharge Planners, Physicians—as well as community organizations and partners. Working closely with internal Admissions Teams, the Liaison/Marketer ensures all clinical and financial information is accurately communicated and assessed.
