We’re looking for someone who knows the local healthcare market, enjoys building relationships, and understands how to turn those relationships into home health referrals and admissions. **What You’ll Do** ------------------ * Develop and maintain relationships with physicians, hospitals, SNFs, assisted living communities, case managers, social workers, and other referral sources. * Generate new home health referrals and help grow patient census. * Build and manage a strong referral pipeline throughout Weatherford, Parker County, and the surrounding territory. * Educate referral partners on home health services, eligibility, and the benefits of care delivered in the home. * Identify new accounts and opportunities within the territory. * Consistently work toward monthly referral and admission goals. * Partner with the clinical team to help ensure a smooth transition from referral to start of care. **What We’re Looking For** -------------------------- * **...
