**Physical Therapist (PT) Rehabilitation** **Location:** Lancaster, NH **Job Type:** Contract **Contract:** 17 Weeks **Pay Rate: $ 2,100 To $ 2,350 per week** **Schedule:** Day Shift \| 8:00 AM 4:30 PM **Experience:** 1\+ years of experiance **About the Role** We are seeking a **Physical Therapist (PT)** for a 17\-week contract opportunity in a post\-acute rehabilitation setting. The Physical Therapist will provide patient\-centered rehabilitation services focused on improving mobility, strength, balance, functional independence, and overall physical performance. **Responsibilities** * Evaluate patients' mobility, strength, balance, functional limitations, and rehabilitation needs. * Develop and implement individualized physical therapy treatment plans based on clinical findings and patient goals. * Provide therapeutic exercise, gait training, mobility training, and functional rehabilitation interventions. * Monitor patient progress and modify treatment plans based on clinical response and functional outcomes. * Maintain accurate clinical documentation and collaborate with interdisciplinary rehabilitation and healthcare teams. **Requirements** * Physical Therapist (PT) Qualificatio...
