The Physical Therapist is expected to follow facility core values in administering department services and helping to fulfill all therapy\-related resident care needs. **Duties/Responsibilities:** * Evaluate, plan, and administer treatment to residents and patients with temporary or long\-term disabilities to relieve pain, restore or improve function, and promote healing. * Assured all treatment is delivered in accordance with an established plan of care * Provide clinical support and instruct patients, families, and caregivers * Measures \& records patient’s motor function, strength, and muscle performance * Monitors patient responses to treatments and makes modifications during treatment sessions as necessary * Completes documentation in accordance with company policy and state/federal regulations **Skills:** * SNF/Long term care experience preferred * Curr...
