**Social Work Director** The primary role of the Social Work Director is to coordinate, supervise and facilitate the delivery of Social Work Services, as provided by facility or facility contracted staff, to all residents of the Facility, and in accordance with applicable state and federal regulations and professional standards. This position fulfills the responsibility of direction of the facility’s social work services as required by the CT Public Health Code. **Responsibilities:** * Supervise the facilities Social Work Services department and facility contracted staff consistent with operational guidelines. * Conduct face to face resident interviews and assessments of resident cognitive, mood, behavioral status in addition to other psychosocial factors. * Participate in planning for resident discharges. * Available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed. * Direct and supervise the delivery of medically related social services to all residents of the facility, as well as their family members and significant others as needed. * Collaborate with the facility Staff Development Coordinator and Administra...
